1. Accessory neurovascular bundle enveloped by roots of tooth 48

A small accessory neurovascular canal is completely enveloped by the roots of tooth 48 at the levels of the apical thirds of the roots. This accessory canal would likely be severed upon the removal of the tooth. No significant neural symptoms are expected, but the possibility of increased hemorrhage should be considered.

Right mandibular nerve lingual to tooth 48M+D

The impacted tooth 48 is vertically oriented in the mesiodistal dimension. The residual follicle surrounding the crown of the tooth appears normal. Root formation is complete.

The right inferior alveolar nerve canal courses lingually to the midroot region of the mesial root of tooth 48, and is moderately compressed. The nerve courses lingually to the midroot region of the distal root of the tooth and is moderately compressed.

The adjacent tooth 47 does not exhibit caries, resorption, or displacement.

An accessory neurovascular bundle is completely enveloped by the apical thirds of the roots of the tooth, as described in the previous finding in this report.

Axial views of tooth 48

These axial cross sectional images of tooth 48 demonstrate the lingual passage of the nerve relative to the mesial and distal roots of tooth 48. The cleft in the mesial root through which the accessory neurovascular bundle passes is evident in these images.

Mesiodistal views of posterior 4th quadrant

These mesiodistal cross sectional images of the posterior 4th quadrant demonstrate the vertical orientation of the impacted tooth 48 and the normal residual follicle surrounding the crown of the tooth. The adjacent tooth 47 does not exhibit caries, resorption, or displacement.
