1. Perforation of furcal surface of tooth 46M by access preparation

There is perforation in the coronal third of the mesial root of tooth 46 through the furcal surface of the root. Localized rarefying osteitis is present in the region adjacent to this perforation.

2. Separated instrument fragment in tooth 46ML

A small separated instrument fragment is present in the coronal half of the lingual canal in the mesial root of tooth 46.

3. Distinct apical foramina of tooth 46M

The canals of tooth 46 do not appear to anastamose at a single foramen. Two distinct root apices are present.

These axial cross-sectional images demonstrate the separated instrument fragment in the coronal half of the mesiolingual canal of tooth 46. The localized rarefying osteitis in the furcal region at the coronal third of the mesial root of the tooth is evident in these images. The remaining alveolar bone around the root surfaces of tooth 46 appears normal.

These buccolingual cross-sectional images demonstrate the furcal rarefying osteitis at the coronal third of the mesial root of tooth 46. The superior cortex of the inferior alveolar nerve canal measures 4.2mm and 5.3mm from the mesial and distal root apices of this tooth, respectively. Note the separated instrument fragment in the coronal half of the mesiolingual canal of tooth 46.
