1. Possible surgical ciliated cyst in left maxilla

There is a well-defined, corticated, multi-locular radiolucency located within the anterior aspect of the left maxillary sinus, apical to sites 24, 25, and 26. Smaller, rounded locules are also observed within the left hard palate, palatal to sites 23 and 25. This appearance is uncharacteristic of normal pneumatization, and could reflect cystic degeneration.

The anterolateral wall of the left maxillary sinus appears focally dehiscent at sites 25 and 26, which suggests a history of sinus surgery. Moderate mucosal thickening and an air-fluid level are observed within the left maxillary sinus. The remaining sinus walls also appear slightly thickened and sclerotic, which is suggestive of chronic antral inflammation.

These findings could suggest the presence of a surgical ciliated cyst. Biopsy with histopathologic correlation is necessary to substantiate this interpretation. Until a definitive diagnosis is reached, regional implant placement is contraindicated.

2. Concha bullosa

The right middle nasal concha contains air. This represents a concha bullosa and is an incidental finding with no clinical significance.
