1. Partially healed socket preservation at site 11

A history of socket preservation is present at site 11. The socket preservation materials appear normally osseointegrated in the inferior and palatal aspect of the site. A defect is present in the buccal aspect of the alveolar process, apical to the socket preservation materials. There are no signs of reparative bone formation or socket preservation materials in this region. Small foci of non-integrated socket preservation materials are retained in the overlying buccal soft tissues, which represents a non-significant incidental finding. Minimal augmentation of the alveolar process has occurred and the overall bone density at this site is slightly lower than that of the adjacent trabecular bone.

2. Open margin on tooth 12

An open margin is present in the mesiopalatal, palatal, and distopalatal aspects of the occlusal restoration on tooth 12. Tooth 12 appears normal otherwise.

3. Widened PDL space on tooth 21

There is focal widening of the periodontal ligament space on the buccal aspect of the apical third of the root of tooth 21. The existing obturation is centrally positioned and extends to the radiographic apex. This may represent the presence of an unfilled lateral canal in this region. Moderate horizontal bone loss is present on the mesial, buccal, and distal surfaces of tooth 21. The remaining periradicular bone appears normal. There are no signs of a root fracture.

4. Periapical rarefying osteitis on tooth 22

Periapical rarefying osteitis is present on tooth 22. The buccal cortical plate overlying this region has been thinned, but remains intact. A small amount of endodontic dressing is present in the coronal third of the root of tooth 22 and the remainder of the root canal appears calcified. This appearance is suggestive of persistent endodontic pathology. Mild horizontal periodontal bone loss is present on tooth 22. The remaining periradicular bone appears normal. There are no signs of a root fracture.

5. Normal fixation plates in maxilla

The fixation plates and screws in the maxilla are partially captured. Based on what can be visualized in this imaging volume, the fixation plates and screws in the maxillary arch are fully seated and appear normal.

6. Soft tissue implant anterior to anterior nasal spine

A semi-radiopaque implant is present in the soft tissue anterior to the anterior nasal spine. This implant has a spiral profile from a coronal perspective. In the absence of any symptoms, this is a non-significant incidental finding.

7. Mild mucositis of right maxillary sinus

The right maxillary sinus exhibits mild mucositis, which represents an incidental finding with no clinical significance.
