1. Periradicular bone loss on tooth 37
Tooth 37 exhibits a broad zone of periradicular bone loss that extends from the periapical region of the tooth to the alveolar crest. The root of tooth 37 has C-shaped axial profile, and may exhibit a strip perforation in the furcal region. No displaced fractures are visible in the root of the tooth, and the pattern of rarefying osteitis surrounding the tooth is not specific for a vertical root fracture.

These axial cross sectional images demonstrate the broad-based periradicular rarefying osteitis associated with the mesial root of tooth 37, which is not suggestive of a vertical fracture.
Post-surgical note
Dr. Pakozdi flapped this region and confirmed the presence of a crack on the buccal surface of the mesial root of tooth 37.
