1. Recurrent keratocyst in the posterior left mandible

No dentition is present in the posterior third quadrant, which suggests that a history of surgery is present in this region. A multilocular radiolucent entity extends from site 34 to site 38. This entity has effaced the border of the inferior alveolar nerve canal in the region of the left mental foramen. The buccal and lingual cortical plates have been lost at sites 34 and 35. Mild buccolingual expansion of the alveolar process has occurred at site 37. Many small daughter loculations are present within the inferior aspect of the lesion at site 35 and 36. This radiographic appearance is suggestive of a recurrent odontogenic keratocyst in the posterior third quadrant. The risk of recurrence is high, due to the presence of many small loculations at the inferior border of the entity in the region adjacent to the inferior alveolar nerve canal.

Mesiodistal extent of the lesion: 38.5 mm

The keratocyst in the posterior third quadrant extends mesiodistally between sites 34 and 38. The mesiodistal dimension of the entity is 38.5 mm

This volume rendering demonstrates the dehiscences that are present in the buccal and lingual cortical plates at sites 34 and 35. Multiple loculations are also visible in the alveolar crest at sites 36 and 37.

This volume rendering demonstrates the prominent defect that is present in the lingual cortical plate apical to sites 34 and 35. The multilocular shape of the lesion at sites 36 and 37 is evident in these images.

Axial views of posterior third quadrant

These axial cross-sectional images demonstrate the large number of small daughter loculations that are forming at the inferior periphery of the recurrent keratocyst in the posterior third quadrant. Note the mild buccolingual expansion of the alveolar process at site 37, as well as the loss of the lingual cortical plate in this region. The absence of buccal and lingual cortical plates in the apical regions of sites 34 and 35 is also evident in these images.

These buccolingual cross-sectional images demonstrate the extension of the odontogenic keratocyst to the level of the inferior alveolar nerve canal. Note the presence of multiple loculations within the entity, as well as the minimal expansion of the alveolar process by the lesion.

The well-defined periphery of the keratocyst in the posterior third quadrant is evident in these images, as is the multilocular shape of the lesion in the presence of many small loculations at site 35.

2. Mild cervical degenerative joint disease

Small osteophytes and mild subchondral sclerosis are present in the articular regions of the Atlas and dens of the cervical vertebrae. This is a common incidental finding with no clinical significance that is suggestive of mild cervical degenerative joint disease.
