Lingual orthodontic wire does not require removal in order to access dens invaginatus in tooth 22

The lingual fixation wire in tooth 22 does not require removal in order to endodontically access the region of dens invaginatus at tooth 22. The potential access preparation is shown in red in the adjacent images, and the fixation wire is shown in blue.

Orthogonal views of tooth 22

These orthogonal cross sectional images demonstrate the lingual fixation wire that is present in the cingulum region of tooth 22, as well as the enamel lined zone of dens invaginatus that is present in the cingulum. This zone of dens invaginatus does not exhibit any signs of caries. The root canal system of tooth 22 appears normal otherwise, and no periapical pathology is present.

Axial views of tooth 22

These axial cross-sectional images demonstrate the enamel lined zone of dens invaginatus in the cingulum of tooth 22. The lingual fixation wire in the cingulum region of the tooth is also evident in these images. The remaining alveolar bone surrounding the root of tooth 22 appears normal.

Mesiodistal views of tooth 22

These mesiodistal cross-sectional images demonstrate the normal appearance of the periapical and periradicular bone located mesial and distal to the root of tooth 22. The dens invaginatus in the cingulum of the tooth is evident in these images. There are no signs of a root fracture in this tooth.

Buccolingual views of tooth 22

These buccolingual cross-sectional images demonstrate the lingual fixation wire in the cingulum region, as well as the dens invaginatus in the cingulum of tooth 22. The buccal and palatal cortical plates adjacent to the root of the tooth appear normal. No signs of caries are present.
