Right mandibular nerve enveloped by mesiobuccal and mesiolingual roots of tooth 48: High risk of nerve damage

The mesiobuccal and mesiolingual roots of tooth 48 completely envelop the right inferior alveolar nerve canal. The nerve passes buccally to the distal root of the tooth at the midroot level. The complete envelopment of the nerve by the mesiobuccal and mesiolingual roots of the tooth increases the risk of iatrogenic nerve damage upon extraction of the tooth. Tooth 48 is mesioangularly impacted and the roots of the tooth exhibit hypercementosis.

Buccolingual views of tooth 48

These buccolingual cross-sectional images demonstrate the passage of the right inferior alveolar nerve canal through the mesial furcation of tooth 48, as well as the buccal passage of the nerve relative to the distal root of the tooth.

Mesiodistal views of tooth 48

These mesiodistal cross sectional images demonstrate the hypercementosis that is present around the roots of tooth 48.

Axial views of tooth 48

These axial cross sectional images demonstrate the complete envelopment of the right inferior alveolar nerve canal by the mesiobuccal and mesiolingual roots of tooth 48.

Left mandibular nerve enveloped by 38MB+ML: High risk of nerve damage

The left inferior alveolar nerve canal passes through the mesial furcation formed by the mesiolingual and mesiobuccal roots of tooth 38. The nerve is completely enveloped by the mesiobuccal and mesiolingual roots of the tooth and is mildly compressed. The nerve then passes buccally to the apical half of the distal root of the tooth and is moderately compressed by the distal root. Tooth 38 is mesioangularly impacted but has not caused any damage to the distal surface of tooth 37.

Buccolingual views of tooth 38

These buccolingual cross-sectional images demonstrate the complete envelopment of the inferior alveolar nerve canal by the fused mesiolingual and mesiobuccal roots of tooth 38, which significantly increases the risk of iatrogenic damage during extraction.

Mesiodistal views of tooth 38

These mesiodistal cross sectional images demonstrate the hypercementosis that surrounds the roots of the mesioangularly impacted tooth 38.

Axial views of tooth 38

These axial cross sectional images demonstrate the passage of the left inferior alveolar nerve canal through the mesial furcation of tooth 38, as well as the fusion of the mesiolingual and mesiobuccal root apices of the tooth around the canal. The buccal passage of the canal relative to the distal root of the tooth is also evident in these images.
